Electronic Health / Medical
Record (EMR) - Progressive and
Technologically Advanced
In order to protect the security of your
personal health information and ensure
optimal continuity of care between our
Physicians and each office, we have
implemented an Electronic Health / Medical
Record system.
Appointments
- We utilize an automated phone system
to quickly direct you to our central
scheduling department to start the appointment process. Patients are seen during regular
office hours and by appointment only, except
in emergency circumstances. To schedule an appointment, you can call
772-398-9911 for Port St. Lucie, 772-464-6055 for Fort Pierce and 863-357-7791 for Okeechobee. Once you hear the automated greeting, press 1.
Occasionally, our office schedule must be
altered due to the unplanned arrival of a
seriously ill or surgical patient. Your
patience and understanding is appreciated.
The Practice has a "No-Show" policy, which
requires twenty-four (24) hours notice if
you must cancel your appointment. If you
fail to abide by the policy, you will be
charged $25.00 per occurrence.
Privacy Notice
- In accordance with the Health Insurance
Portability and Accountability Act of 1996,
patients of this Practice are entitled to
the greatest degree of privacy possible.
This office will strive to ensure that
patient information is used only for
authorized purposes as agreed to by the
patient.
Our Consent/Authorization form states that
the release of medical information to any
insurance carrier, other entities directly
associated with the offices of Drs. Berghash
& Lanza, P.L., primary care provider and/or
referral physician in connection with
treatment is authorized. No other
disclosures will be made without written
authorization from the patient or guardian.
Patients are advised that they have a right
to review their medical files upon
reasonable notice to the Practice and during
normal business hours and to make comments
to the same. These requests must be made in
writing and directed to the "Compliance Officer"
Emergencies -
Drs. Berghash & Lanza, P.L. provide emergency
patient coverage twenty four (24) hours a
day, seven (7) days a week.
In the event of an emergency during
non-business hours, call our office number
and give the answering service your name,
telephone number, and brief summary of the
emergency. The physician on-call will return
your call was quickly as possible. The answering service can be contacted

219-2430. If for whatever reason you cannot
reach one of our doctors, go directly to the
nearest emergency room.
Refills of pain or other routinely used
medications will only be handled during regular
business hours.
Insurance / Payment
- Drs. Berghash & Lanza, P.L. are
participating providers for the insurance companies listed here.
Insurance plans
vary greatly and each plan has its own
policies, eligibility requirements, and
restrictions. Please present your insurance
card at the time of your visit to help us
determine your financial responsibility.
If we are a participating provider with your
insurance plan, we will ask you to pay your
co-payment, deductible, or co-insurance
amounts at the time of service and we will
file the necessary claim to your insurance
company.
If we are not a participating provider with
your insurance plan, you are expected to pay
in full at the time of service. We accept
cash, personal checks, MasterCard, Visa,
America Express and
Discover. We do not offer patient financing.
Messages - Our
office staff is trained to answer many
routine medical questions. All routine
questions received throughout the day are
directed to our message system. These
questions will then be reviewed with the Physician and you will receive a
reply from one of our medical assistants by the end
of our business day.
Prescriptions
- Medications will not be prescribed for any
condition that has not been evaluated. When
you need a refill on routine medications,
please call the office leave a message on the medical assistant's voicemail. Provide your
name, the telephone number where you can be
reached if needed, your pharmacy name and
telephone number. Routine medications will
be refilled without an appointment if you
have been treated for that condition within
a three (3) month period. If you have not
been evaluated within three (3) months,
please call the office for an appointment.
All prescription refills will be called into
your pharmacy by the end of the next
business day.
Scheduled Surgeries
- Many surgical procedures can be performed
on an out-patient basis. When possible, we
will schedule these cases at an out-patient
surgery center. This helps reduce the
patient’s out-of-pocket cost.

Each patient has the right to receive services in the location of their choice. Our ability to accommodate your choice may be influenced by whether the Physician is on-staff and/or credentialed at that facility.

Our Surgical Coordinator will obtain any
applicable pre-authorizations or referrals
on your behalf, and will schedule the
necessary pre-operative appointments and/or
tests associated with your surgery.
